File a Complaint

If you are a customer of a financial institution and have a complaint, the Utah Department of Financial Institutions (DFI) or another regulatory agency may be able to assist you. The DFI does not regulate all financial institutions in Utah.

To avoid unnecessary delays, we strongly encourage you to begin by determining which agency(s) may have authority to process your complaint. This is a two-step process. Step 1 will take you to a different page on our website. After you complete Step 1, return to this page to complete Step 2.

Step 1) Click here and enter the financial institution’s name in the “Find by Name” field. Find the institution’s name in the result list, and then note the financial institution type shown under the name. (Hint: the types are shown on the following chart.) If the financial institution does not appear on our list, call us at 801-538-8830 for assistance.

Step 2) Look at the financial institution type in the following chart to find the appropriate regulatory agency and contact information.

The DFI only accepts written complaints. Verbal complaints cannot be accepted or reviewed. Your written complaint may be a letter, email, or this complaint form. Emailing your complaint to complaints.dfi@utah.gov will ensure the fastest service.

The complaint form is an Adobe Acrobat form that needs to be filled out in Adobe Acrobat Reader, not your browser’s PDF viewer.

If you cannot email your complaint form, you may complete it by hand and submit it by one of the following options

We will email written acknowledgment to you within seven (7) business days of receipt of your completed complaint form. If you did not provide your email address, we will mail a letter to you.
